This is a great idea!! You might add a space for “general comments or suggestions”?
Yeah, I wish I could, but the “free” version I used at Survey Monkey limits me to just 10 questions……thus, if there are any more comments or suggestions I suppose they’ll just have to make a comment here (which of course wouldn’t be anonymous)
